Thank you for your interest in the "Help Soldiers Sleep" Community Outreach Program

We appreciate your interest and willingness to distribute the Help Soldiers Sleep (HSS) CDs and Questionnaires to your active and retired military soldiers.  The short Questionnaires help us determine the effectiveness and value of this program.

Below are easy to follow instructions to help insure a successful program:

1.  Assign a HSS Contact Person

2.  Determine the Number of CDs & Questionnaires Needed

3.  Schedule a Delivery Date

4.  Confirm with HSS that the CDs have arrived

5.  Distribute CDs and Questionnaires to soldiers

6.  Contact Person follows-up with soldiers to collect Questionnaires

7.  Contact Person either mails the Questionnaires or schedules a pick up time with the HSS Representative

8.  Contact Person receives a Follow-Up Survey from HSS to determine the effectiveness of the program
